RP Donor Recognition Reply Form

Donor Recognition is an opportunity for the chapter to thank the previous year’s Honor Roll level donors.

You will receive items to hand out for your chapter donors who gave $500 or more.
Individuals who gave $150 - $499 received their items directly.

This form must be completed by October 15 and the Recognition Event must be held by December 31.

Please submit the below information to receive your items and qualify for PAOE points.
